While filling the IEPF 5, There are only 15 rows for claiming unclaimed dividend, We have unclaimed amount for 18 years, Pls guide should we skip some amount or club some amount.

Kindly help with below queries on claiming Reliance shares & Dividend from IEPF.
1)The Indemnity Bond generated by filling IEPF5 web form contains only first holder name as claimant. I am the joint holder. I should put my signature along with claimant (first holder)?
2) My dividend is for multiple years. In the IEPF Indemnity Bond, can I write ‘multiple years’ in the ‘Financial Year’ field?
3) What should I select from the below? (Dividend on Equity / Preference shares / matured deposits / matured debentures / application money due for refund /interest accrued on……………………./Dividend credited to IEPF under Companies Act, 1956 or Companies Act, 2013/ Dividend credited to General
Revenue Account under Companies Act, 1956 / others, specify………….)
